What is the Yellow Dot program?
Imagine you're in a serious car crash.
Would you be able to tell first responders about your medications, medical conditions, or who to contact?
The Yellow Dot Program helps you share this critical information, even if you can't speak.

How it works
You fill out a Yellow Dot form & put it in your car's glove box
You place a Yellow Dot sticker in your car's rear window
First responders look for the sticker in the event of a crash
How to get set up with Yellow Dot in your vehicle
You need three items:
- Yellow Dot form
- A photo of yourself
- A Yellow Dot sticker
Get and fill out the Yellow Dot form
- Make sure you put all of your current medications and medical conditions on the forms.
How to get the form
- Download the form here
- Pick up a form, packet, and sticker from one of these locations:
Take and print a photo of yourself
- Make sure it's a photo from which you can be easily identified (no hats or sunglasses)
Place the medical form with your photo in your car's glove box
Place a Yellow Dot sticker in your car's rear window
